This book reflects a collaborative project in which the exchange of ideas and materials between contributors has been far greater than in a conventional ‘edited’ book. This integrated approach was evident from an early stage when the draft chapter 1 was circulated to all contributors for comment. It was also the focus of a seminar to explore with contributors the underlying ideas and rationale.
